PDN Nurse (Laredo & Zapata)

Position Overview

We are seeking Private Duty Nurses (RN/LVN) to provide specialized in-home care for new low-acuity and high-acuity pediatric patients in the Laredo and Zapata area. This role includes a variety of shifts that you can select on the application. Responsibilities include monitoring the patient’s health, documenting medical changes, collaborating with healthcare providers, and ensuring a safe, nurturing environment. High-acuity patients require more detailed care such as ventilator/tracheostomy care (if applicable), responding calmly and effectively in urgent situations and administering critical interventions. Ideal candidates will have pediatric experience, strong communication skills, and a heart for supporting families through dedicated one-on-one care.

This weekday, night shift role requires advanced clinical skills to manage complex medical needs, including continuous monitoring, ventilator/tracheostomy care (if applicable), and administering critical interventions. Responsibilities include coordinating closely with physicians, documenting changes in condition, and ensuring strict adherence to care plans. Ideal candidates will have prior high-acuity pediatric experience, and the ability to respond calmly and effectively in urgent situations.

RN Case Manager

Position Overview

We’re seeking a compassionate, Texas-licensed RN with at least one year of nursing experience to join our Case Management team. In this role, you will provide critical support for pediatric patients receiving Private Duty Nursing (PDN) services in the home. Ideally possessing a background in PDN, you will utilize your strong clinical and communication skills to serve as the bridge between our bedside nurses, families, and physicians. This Full-Time, Monday–Friday role (with some on-call) requires a professional with excellent time management who is dedicated to ensuring quality and continuity of care for our “Little Warriors.”

Coordinate comprehensive care for medically complex pediatric PDN patients within their home environment.
Perform routine in-home assessments and supervisory visits to monitor patient progress and nurse performance.
Develop and manage individualized care plans, ensuring they are strictly followed and updated as medical needs evolve.
Provide clinical support and guidance to bedside nurses, acting as a resource for complex medical questions.
Collaborate closely with families and physicians to ensure all therapeutic and medical goals are met seamlessly.
Maintain precise documentation of patient status and care coordination in accordance with state and federal regulations.
Oversee quality and continuity of care, troubleshooting clinical or scheduling challenges as they arise.
